Fees
The board has established the following fees for testing and licensure.
- Application for Licensure — $500
- Reciprocity Application — $500
- Licensure fees Applicants granted licensure will pay — $250
- Licensure fees Reciprocity — Applicants granted licensure will pay $250 on licensure approval
- Renewal Fees (annual) — $150
- Replacement license — $10 and a written request
- Corporation Registration —- $50
- Corporation Annual Renewal — $10
- PM Lexis Fee direct to PM Lexis.
All fees are non refundable.
