To be eligible, an applicant must:

Mothers, infants & children who meet WIC criteria and are income eligible on Title XIX Medicaid, TANF and SNAP (formerly Food Stamps) automatically meet income guidelines for WIC and need to show proof of Medicaid, TANF or SNAP information.
Salaried or Hourly Wage Earners need to provide proof of income with most recent U.S. 1040 income tax form or the average gross income of the two most recent check stubs and calculated for annual income.
Self-Employed or Farmers use net income; gross sales or income minus operating expenses for annual income.
